What's been said is absolutely true. But another thing I think is going on with people who have this complaint is subconscious. It may be a fear to do things "wrong", it may be a feeling that a setting you've heard is somehow sacrosanct, it may just be that you have to concentrate too hard...

I agree with what has been said. Changing the relative temperature levels of the body and mouthpiece so that the body shrinks and the head grows slightly is probably the way to go. You might sit the bottom of the body in a glass of ice water for several minutes, then dunk just the mouthpiece in hot ...
